Thyroid Medication Options: T3, T4, and DTE Explained

You finally get a diagnosis. You start the medication. And then you wait for the fog to lift, the energy to come back, the scale to budge. But weeks or months later, you still feel like you are dragging yourself through the day. If that sounds familiar, please know you are not imagining it, and you are not alone. Your thyroid medication options may simply need a second look.
Conventional treatment usually begins with a single hormone, T4. This approach works well for some patients, yet others are left wondering why their labs say “normal” while their body says otherwise. Understanding the full range of thyroid medication options, including T3, desiccated thyroid extract, and various compounded formulations, can help you and your provider find what actually fits.
How Thyroid Hormones Keep Your Body Running
Your thyroid acts like the body’s power plant. It produces two key hormones that regulate metabolism, energy, heart function, and mood:
- T4 (thyroxine): The storage, or inactive, form of thyroid hormone. About 80 percent of T4 must be converted into T3 in the liver, gut, and muscles before it can be used.
- T3 (triiodothyronine): The active form that fuels your cells and drives energy production.
A healthy thyroid secretes these hormones in roughly a 14:1 T4-to-T3 ratio. When conversion falters due to stress, nutrient deficiencies, gut imbalances, or liver dysfunction, you may still feel hypothyroid even if your lab numbers appear normal.
Common Symptoms of Hypothyroidism
Hypothyroid symptoms tend to build slowly, so they are easy to dismiss as simply being busy, aging, or run down. Watch for a cluster of these rather than just one:
- Fatigue that does not improve with rest
- Feeling cold when others are comfortable
- Unexplained weight gain
- Dry skin, thinning hair, or brittle nails
- Constipation
- Muscle aches or stiffness
- Brain fog or trouble concentrating
- Puffiness in the face or limbs
- A hoarse voice or slower heart rate
- Low mood
- Irregular or heavy menstrual cycles

Why T4-Only Thyroid Medication Options Fall Short for Some People
Here is the part most people are never told at diagnosis: taking T4 does not automatically mean your body can use it. Levothyroxine and similar brand-name drugs supply T4, the inactive storage form of thyroid hormone. Roughly 80 percent of it must convert into T3, the active form your cells actually run on, before it does any work.¹
Think of T4 as a letter written in a language your cells cannot read on their own. T3 is the translated version. Your body needs a translator- the conversion process that happens in the liver, gut, and muscle tissue – to turn that letter into something your cells can actually understand and act on. When the translator is overworked or missing key tools, like selenium or zinc, the message gets delayed even though the letter arrived just fine.
So, when your bloodwork comes back “fine”, but you still feel exhausted, cold, or foggy, the issue may not be how much hormone you are taking. It may be how well your body is translating it. That is one reason a single thyroid medication option simply does not work the same way for every person.
Exploring T3 and Desiccated Thyroid Options
Desiccated thyroid extract, or DTE, comes from porcine (pig) thyroid glands and naturally contains both T4 and T3. Armour® Thyroid is the most recognized brand. Each grain, or 60 mg, provides approximately 38 mcg of T4 and 9 mcg of T3, a ratio of about 4:1, compared with the human thyroid’s natural output of roughly 14:1.²
For anyone whose body struggles to convert T4 on its own, having some active T3 already in the mix can make a real difference day to day. Many people describe finally having enough energy to get through an afternoon without a crash. In fact, one clinical comparison found that a large share of participants preferred desiccated thyroid extract to levothyroxine for symptom relief.³ Even so, DTE is not without its challenges:
