PHYS PREF EYE RENEWAL CREAM

This prescription has been compounded by Physicians Preference Pharmacy on physician’s orders.

DRUG:

PHYS PREF EYE RENEWAL CREAM  is comprised of  Caffeine, Vitamin K, Vitamin E,  Vitamin C, Hyaluronic Acid, Co Q10, Chrysin, Grapeseed oil, and GHK-Cu in a paraben free cream base.

USE:

PHYS PREF EYE RENEWAL CREAM is a topical anti-oxidant cream.

DIRECTIONS:

PHYS PREF EYE RENEWAL CREAM is to be used under the eyes one to two times daily.  It may be applied under a moisturizer and make-up.

CAUTIONS:

Keep out of eyes. If in contact with eyes, flush with copious amount of water for 15 minutes, separating the eyelids with fingers. If irritation persists seek medical aid.

POSSIBLE SIDE EFFECTS:

If redness or irritation occurs, discontinue use.

STORAGE:

Upon receipt, store at room temperature between 59 and 86 degrees Fahrenheit away from sunlight and moisture.

DESCRIPTION:

Clear airless pump with silver cap filled with yellow cream.

DISPOSAL OF UNUSED MEDICATION:

Do not flush unused medication or pour down sink or drain.
